A total of 245 cases of surgical patients with duodenal ulcer who were admitted to the Department of general surgery, St. Benedict Hospital, Pusan were clinically reviewed from January, 1987 to December, 1989.
1) Of 245 cases who were treated by surgical intervention, perforation was the most common complication(48.1%), and the other complications were intractability(20.4%), obstruction(18.7%), and hemorrhage(12.6%).
2) In age distribution, the highest incidence was 6th decade of life(22.4%) and the majority of cases occurred between the age of 3th decade and 7th decade of life.
3) The complications of duodenal ulcer occurred 6.65 times more often in male than in female.
4) The duration of symptoms of more than 3 years was noted in 48.2%, and that of less than 1 month noted in 29.8%.
5) The subdiaphragmatic free air was noted in 71.2% of perforated duodenal ulcer on erect simple abdominal X-ray.
6) In the operated patients, the sites of ulcer were as follows; 71.9% in duodenal bulb, 21.2% in pyloric channel, 2.9% in postbulbar region, and 4.1% in combined gastroduodenal region.
7) Regarding the operative procedures, the most common operative procedure was bilateral truncal vagotomy and pyloroplasty(47.3%) and the other procedures were bilateral truncal vagotomy and hemigastrectomy(41.6%), subtotal gastrectomy(4.1%),
primary
closure(2.4%), bilateral truncal vagotomy and gastrojejunostomy(1.6%), highly selective vagotomy (0.08%), and simple gastrojejunostomy(0.04%).
8) Postoperative complications were noted in 18.4% of all surgical cases; the most dommon complication was wound infection(4.5%). The highest postoperative complications was noted in patients with bleeding(58.1%).
9) Postoperative death was not noted.
